For those who have been selected for the 2008-2009 BNSC competitive teams, please follow the directions listed below:


1. Print off each of the following forms

• Club Registration Form
  
• Medical Consent
  
• Assessment & Handbook Agreement (Handbook available here)
  
• Code of Conduct
• US Club Soccer Form
  

2. Fill out and complete each form.

3. Please give completed forms to your team manager , along with the following; state issued birth certificate (NEW PLAYERS ONLY),  uniform sizing (NEW PLAYERS)  & checkbook (if not paying online) .
